Even though I play nearly daily and am in the industry as an affiliate as well, I don't really feel I can contribute very much to this topic.
But this is what I've got...
For my top 5: At the moment (and for quite a while) I only play regularly at 2 casinos...
1. Videoslots is by far the best casino experience for me as a PLAYER (but close to worst as an affiliate - more on that later)
I have never had any hassles with them, they give generous bonuses, CS are on the ball, they have a HUGE range of games (even though I tend to stick to just 20-ish regulars), payouts are FAST, and above all - I seem to have lots of good luck while playing there!
2. Casino Extreme is an RTG casino which has always been one of the best on this software IMO. Probably the best thing about them, is hassle-free INSTANT withdrawals (via Bitcoin).
From previous experience as a player, and reviews I have seen as part of my job, I think these are also worth a mention...
Vera and John used to be one of my favourites... until they pulled out of the UK
I presume they are still going strong in the EU - if so, I would recommend them.
All British. Only played there a little bit, but from my experience and what I've read, they seem to be top-notch.
3Dice. Used to play there a lot back in the day, despite the fact they were very "affiliate unfriendly". For such a tiny casino with a very small range of unique games, they have frankly surpassed my expectations of longevity and amazing CS.
Best CS? I very, very rarely have the need to use CS at any casino - so I can't really say who is best.
Best Group? I would have to say

Despite the awful Club World fiasco they had to endure, these guys came out the other end still offering the same great level of player and affiliate fairness
Best Manager? The only one I know really is Enzo from 3Dice - so I'll give him my vote!
Best thing to happen to a player = Winning!
Worst thing to happen to a player = Losing
Biggest disappointment? Yeah - still Videoslots Affiliates I'm afraid, for continuing to short-change their "marketing partners".
When they started out in 2013, their reporting system was basic, but fair. Then they removed half the data fields so that affiliates couldn't see what was going on - that was when they snuck in an extra 25% deduction off GGR so their affiliates got a smaller slice of the cake. Finally, they produced a much more detailed reporting system, but it's full of glitches and errors where the figures simply don't add up.
Not only is this frustrating, but it also makes absolutely no sense for themselves as well. If they treated affiliates as fairly and honestly as their players, they would get much better exposure and therefore dramatically increase their player numbers IMHO. Such a shame.
