Maybe you are not using the correct codes to access the InetBet fax machine. The correct code MAY depend on the SENDER's location, not just where the recipient is. The UK phone system was once like this, and I get the impression there are still some lingering oddities in paerts of the US phone network due to it always having been run by several separate companies.

An alternative would be to scan the form into an image file, a JPEG will be fine (ensure the resolution is good enough such that details can be read), and E-mail it to support to pass on. Later, follow up to ensure it has reached the right department.
Some casinos have developed methods to directly upload documents, thus avoiding E-mail altogether. It is surprising how rare this still is. I first came upon this at Click2Pay (they at least got ONE thing right:rolleyes:), but have seen something similar at some casinos.
The big vendors, such as MG and RTG should consider adding a lobby facility such that players can upload an image to casino support. This could allow them to send screenshots, enter promotions where a picture really helps, as well as add & update their documents whenever they need to, not wait for a withdrawal to be delayed.
I expect MG will be the first to do this, their lobby options seem very close to achieving this already, plus operators can use an interactive lobby webpage under the "homepage" option. Other vendors would probably need their operators to use a webpage, although Cryptologic could add a link in the panel available for promotions within the software.
