Feedback Problem with PAB form Access

Not sure if I'm posting this is the correct place so sorry upfront if it needs to be moved.

Can't seem to access the PAB form

When I click the Pitch a Bitch FAQ tab it takes me to the FAQ page.
Then Click on item
3.2 Where to find the form it takes me to this page

When you click the hyper link for the actual form you need is HERE it takes you to this page

In the middle of this screen it says CM members can submit complaint HERE. clicking this link takes me to this error.

Thanks! For some reason, I'm guessing during the recent upgrade, the PAB form was turned off for everyone except admins and moderators. It's on now, so please PAB freely.
