In your many years, have you seen anyone Banned, even in a nice way, who almost always takes money off the table games, without using any Bonus, or cheating in some manner? Does there come a point where the big boss man says, " Player XXXX is so far ahead, unless he/she changed their play style completely, and started betting max table limits, we will not be getting our money back", and asks them to leave?

or, Do they they ask him permission to use him as advertising to let other players know, "See, XXX Wins, and so can you". ?

As for the winning players. I can honestly say that I have never closed an account or banned someone simply for winning. I don't doubt that some operators see this as the only solution to someone who consistently wins and it is ultimately their product, so they have every right to close an account (so long as all winnings are paid obviously). There are better ways of dealing with it though, like using the winnings for promotional purposes like you said. People have to win in a Casino, otherwise what is the point of playing?
