I just wanted to add that a number of the questions raised in this thread have been answered in the ICE report here:

One thing I should add concerns RTG software and the ability to deselect certain games when a bonus is used. There are different versions of the RTG software that is periodically updated and launched. A new version should be coming out in the next several weeks that should allow operators to deselect non-qualifying games for certain bonuses. So if you have accepted a slots bonus, you won't be able to play Blackjack with it. This will eliminate bucket-loads of problems - I wish they did this 10 years ago.

Unfortunately, the software won't be able to automatically limit bet sizes when an operator creates a bonus that has bet size limits. I guess no one has thought of asking the developers to implement this sort of thing. I'll shoot off an email to see if this can't be done.
