When my computer died recently I had to reinstall the casinos on my new one. If you don't remember your username(s) you used to have to write support or get on chat to get it. But MG has this cool thing now where if you don't remember your account name/number you click the link, it sends you a mail with a link to a secure page where you can find your account number and/or change your password.
I went to install Ruby Fortune and when I clicked to get my username I discovered that I had 2 real accounts, one hadn't been accessed since 2006. I went to chat and told them, they looked and said, "Huh, you're right, we'll close the old one."
So if it's just a simple mistake, most casinos don't have an issue and will close old accounts. I think the main issue that they have is people signing up with multiple accounts and using big SUBs or ND chips. If you've got multiple accounts and they've all been opened within a particular time frame, or if you've taken signup bonuses or free chips in more than one account, you're screwed.
