These rules are standard, but they should not expect the impossible from players. You have done all physically possible, and yet this is not good enough. There ARE other ways to verify the identity of a player, and allthey need to check is the statement, and see it matches the deposits from the card. If you had stolen the card, you would not have the statement, and it would be highly unlikely you could furnish the relevant utility bills etc.
Asking the bank to write a letter is unreasonable from a privacy point of view, as this would require you to inform your bank why you need such a letter, which would mean telling them it is for an online casino.
The lost card is history, you can never get an image of it now, and if all the other details match, the casino should be satisfied with alternative proofs of identity. They could also ask you to sign an indemnity form listing all the deposits from the lost card, including a declaration the card was lost and replaced by the bank. This would leave your bank out of this, and would mean you could keep this habit to yourself
If they processed the withdrawal back to the lost card, and you still have the bank account, the bank should still be able to receive this for you, although check with them first.
Scan the new card, and any future cards you use at casinos, as this is a fairly standard request, and will make losing them less of a problem when you cash out.
If you are worried you will lose the 2000 in the meantime, ask the casino to lock your account until this withdrawal can be processed (this might actually encourage them to speed things up a little, they will not be able to persuade you to lose back the 2000 to end the problem).
This problem could arise at ANY casino you have used the lost card at, as well as those in the same group. If joining a new casino with the new card, you get a clean start.
There have been cases where casinos have asked for images of old expired cards, and this has been a problem because they have been destroyed by the holder when the new one arrived, again common sense left the building, and there were arguments about getting these long destroyed cards, or their long shredded statements, scanned and sent to the casino.
Those who have played a while have probably scanned and archived images of EVERY card they have used at online casinos, just in case they are asked to produce it later (I soon decided this was a good idea, and I keep scans of all my old cards in an archive - just in case).
