OPA Complaint form Nit

I just filled out one of these for the first time recently and had one small comment.

The form asks:
Did you deposit?
Did you get a bonus?
If so, how much?

To me, it certainly looks like "how much" applies to the bonus question on the line above. After the form is processed, it turns out that the question was actually asking how much was deposited.

ha ha It was a trick question. No actually I have no idea why that happens. Just tested it myself and your right.

I will have the Tech guy fix this soon as possible.
