The notary public stuff is utter BS and a stalling tactic. It's really expensive and proves exactly nothing, just that the notary public has seen the docs (not that they're legit). My advice is to set a wagering limit to $1 under My Account -> Responsible Gambling, this will ensure you don't lose anything back. Then go get the docs, send them in and steer clear from this casino forever.
I have got the same request from a Progressplay casino last year, but I haven't got around to actually get the docs signed since it will cost me €150+ and the withdrawal is for €500. The reason for their request is because I deposited with my MC debit card, and that they don't have that as a payout option. Will probably do it one day when I have the energy.
