Complaint : I cashed in $60 on 8/2/2001. $30 of this amount was my own deposit and it was refunded back to my credit card but I am still waiting for my $30 winnings check. I logged into the casino today and it is still available for reversal.
How Resolve: Please help me get my $30 winnings check from this casino.

I, , give permission for Casino 388 to release all information regarding my account to the OPA.I also agree not to settle this claim without first notifying the OPA of my intentions to do so. I further agree to be truthful and honest in my communications with the OPA. I understand that by misleading the OPA in this matter could result in my membership being canceled in the OPA. I further agree to release all terms of any settlement agreement with the OPA I further agree to reimburse the OPA for any out of pocket expenses concerning this complaint.
