For deposits I use a prepaid Mastercard from Moneymart. So far it works everywhere, fingers crossed.
For withdrawals, I use Moneybookers at
RTG and Microgaming. You can then either withdraw from Moneybookers via bank wire or I request a cheque, and I usually have it within 8-9 business days.
I also use Payspark for Micro withdrawals. I've used them for years, and think they're great. They have an ATM card, so as soon as the money hits my account...I go and withdraw it. The only bummer is that 95% of my play is at 32Red/Dash, and they don't use Payspark. So for them, I use Moneybookers to withdraw.
Ecocard is also available for Canadians. I have an account, but have never used it....too complicated for me. And the only way to get your money from Ecocard is via bank wire (I think). I don't think they offer a cheque withdrawal.
There are also some new payment options on the horizon, but there's no set time as to when they will be implemented.
